The world is still reeling from the shocking death of wrestling icon Hulk Hogan, and now police in Florida are investigating whether a surgical mistake may have played a role in the tragedy.
The Clearwater Police Department confirmed Thursday that its investigation into the WWE legend’s passing is still ongoing, telling TMZ Sports:
“We have been in communication with family members — including his son, Nick, and daughter, Brooke. The unique nature of this case has required us to interview multiple witnesses and seek medical records from a variety of providers, and our detectives continue to do that. All of this takes time. The Hogan family has been appreciative of our efforts and our communication with them.”
A Troubling Surgery Detail
Hogan’s widow, Sky Hogan, revealed that her husband’s phrenic nerve was “compromised” during a recent medical procedure. The phrenic nerve is essential for stimulating the diaphragm, the muscle that controls breathing.
An autopsy has already been completed, but Sky declined to reveal the findings. She also confirmed that Hogan’s body has not yet been cremated, leaving room for further medical review.

What Happened the Day Hogan Died
According to law enforcement sources, the police report from the day Hogan died on July 24 is being tightly restricted. But investigators say an occupational therapist was inside Hogan’s Florida home when the emergency unfolded.
The therapist reportedly told responding officers that Hogan may have been the victim of medical malpractice — specifically, that a surgeon may have accidentally severed his phrenic nerve during either a heart surgery or a neck operation.
Crucially, sources say Hogan did not appear to suffer a typical “chest-clutching” cardiac event. Instead, he simply stopped breathing. It was Sky who noticed his breathing had halted and dialed 911 for help.
Evidence on Body Cam
We’re told officers discussed the occupational therapist’s claim at the scene, and their body cameras likely captured both those conversations and the original statements from the therapist. This evidence could become central in determining whether malpractice charges are pursued.

Hogan’s daughter, Brooke Hogan, has already called for a full autopsy, underscoring the family’s determination to understand exactly what went wrong.
